TECHNOLOGY TRANSFER PROGRAM (TTP) REQUIREMENTS
The VOSH/International Technology Transfer Program (TTP) recognizes request for equipment if it meets the following criteria: · Is a recognized charitable NGO or an institute of higher education. · Provides references and credentials. · Is non-sectarian. · Includes name, address, email, fax and telephone. · Has Custom’s permission to receive donation. · Provides copies of paperwork sent via electronically, email or fax. · Confirms smooth passage across borders until it arrives at final destination.

| Process for Review of Technology Transfer Application
1. No application will be accepted without co‐sponsorship by a VOSH or SVOSH chapter or the VI board. 2. Equipment will be distributed in order of the following priorities: a. Optometry schools b. Non‐profit medical clinics c. Medical or outreach teaching clinics with optometry providers on staff. 3. All electrical instruments are assumed to run on 110 voltages. Any transformers or surge protection are the responsibility of the applicant. 4. The applicant is responsible for all shipping charges from the VOSH warehouse in Florida. Items will not be shipped unless all arrangements are made to the satisfaction of the TTP committee, including but not limited to: a. Arrangements to pass customs and pay all applicable duty and taxes b. Notification as to who has authority to sign for the equipment once in country c. Identification as to who is responsible to transport the equipment to the clinic site from the entry point into the country. 5. The VI board has final approval over all TTP committee recommendations. A completed application will be presented by the TTP committee at the next VI board meeting or conference call for board approval. 6. All equipment is shipped with the understanding that the equipment will be used at least partially for humanitarian relief as well as ophthalmic education. 7. The applicant will provide the TTP committee with a letter of receipt. 8. After VI board approval, the TTP committee will direct Max to send requested items to applicant. Max then confirms with a shipping inventory. Applicant should respond with a letter of receipt. The TTP committee will put the application, the shipping inventory, and the letter of receipt together for future reference. Projects like Nicaragua would require the board member in charge to approve the request and send directions to the TTP Chair. |
